Dog/Puppy Adoption Form

Adoption Fee for Adult Large Breed Dogs: $400.00

Adoption Fee for Adult Small Breed Dogs: $500.00

Adoption Fee for ALL Puppies: $500.00

Includes: Deworming, health exam, distemper/parvo vaccines (3 for puppies 2 for adult dogs), rabies vaccine, spay/neuter, tattoo

Process

Completely fill out an adoption application. An incomplete application will delay the entire process. Please include the name(s) of the dogs/puppies(s) you are interested in.
Once the application is received, it will be reviewed by a member of our team. At this time, one of our team members will contact you in regards to your application, following which your references will be checked.
Your application looks good and your reference checks are completed? A member from our team will be in contact with you to set up a meet and greet!
The meet and greet has taken place and you feel you have found your potential new family member! Your application will be forwarded to our home check team and they will be in contact with you to set up a home check. A home check is nothing to be worried about, it is simply to ensure that the animal being adopted will be in a safe environment.
